Please complete the below form entirely and return it to the Upper Gwynedd Police Department Attn: Ofc Whisler or email to bwhisler@uppergwynedd.org Please fill out a separate form for each dog. This registry will be used to assist police in identifying and returning any loose or found dogs. Records will be maintained for a period of three years.
